extremely important to me. Loss of confidentiality can occur with health insurance companies, and I believe in providing
you a choice to ensure the most beneficial treatment for you. If you have an insurance other than
Medicaid or Tricare which provides coverage for this service, I am happy to assist in signing claims forms that you provide.
If you chose to use an insurance I do not accept then you are responsible for mailing all claims to the
health insurance company and tracking your reimbursement. You are responsible for the full fee regardless of your insurance
company’s reimbursement policies.
